Referral Information
Effective from 1st February 2010
From February 2010 the Pathway Development Service will only be accepting referrals from the Regional Forensic Case Managers and Probation Officers in Yorkshire and Humber.
(Please note that if you have a Service User who you think requires specialist placement outside of mainstream adult mental health services please contact the PDS to discuss a referral for assessment or review)
The service operates 3 distinct functions:
1. Assessment
-The purpose is to gain a comprehensive picture of a service user’s journey and provide a formulation and identify realistic recommendations for future care pathways.
-The service user should have a personality disorder diagnosis or presentation consistent with the diagnosis.
- Maximum of 8 weeks with a minimum of 4 sessions offered to the service user referred.
- Regular contact with the referrer regarding the progress of the assessment.
- Meeting with the service user on completion of the assessment to read through the report with them
- Organising and facilitating planning meetings regarding future care and pathways available.
2. Reviews
- The purpose is to focus on those service users who are in secure services or acute services where they are believed to require secure care.
- The aim is to assess current presentation and need, treatment received/is receiving and the progress they have made and make recommendations regarding how current need could be best met in the future.
- The service user should have a personality disorder diagnosis or presentation consistent with the diagnosis.
- The review will take 2-3 weeks and involve 1-2 sessions with the service user and clinical team.
- The report will be sent to the clinical team and forensic case manager (FCM) on completion.
3. Training
- The delivery of the KUF awareness programme to relevant mental health services both statutory and third sector and probation staff across the region commencing in February 2010.
We currently provide assessments of people with personality disorder or traits consistent with the diagnosis who are from the boundaries of the Yorkshire and Humber Strategic Health Authority region. This includes North Yorkshire, West Yorkshire, Humberside, North Lincolnshire, North East Lincolnshire and South Yorkshire. We also assess individuals referred to us by the Regional Forensic Case Managers.
